Your First Visit
Your first appointment is a comprehensive consultation that allows us the time needed to get to know each other. We want to know everything about you including what you do for work, what your hobbies are, and all about your family. We also want to know about your medical and dental history, your diet and lifestyle, any concerns you have, and your oral and overall health goals. We will take the time to answer any of your questions and provide you with a detailed diagnosis and treatment options if needed.
Please bring the following items (if applicable) to your first appointment:
- Valid ID
- Referral slip
- X-rays
- List of medications
- Insurance card
- New patient forms

Insurance & Financing
At Dayton Dental, we believe quality dental care should be affordable and accessible to everyone, regardless of insurance coverage. That’s why we work hard to maximize insurance benefits for those who have it and to help those who don’t by offering affordable care and flexible payment and financing options. You should never have to choose between your other financial obligations and your oral health. We would be more than happy to sit down with you and come up with a plan that works for your budget.
Private Pay
If you don’t have insurance and are paying out of pocket, patient portions are due at the time of service. We accept checks, Zelle transfers, Visa, MasterCard, Discover, American Express, or Apple Pay.
Insurance
We are happy to file your claim for you. You are fully responsible for the total payment of the treatments performed, including any portion not paid by your insurance. Payment for the estimated portion not covered by insurance is due at the time of service.
